Immunization Regulations have changed starting July 1, 2019

All incoming 7th grade students for 2019-2020 will need to show proof of

  • the standard required immunization for school
  • and 1 dose of Tdap (booster for tetanus; diphtheria; and pertussis, also called whooping cough). given on or after the 7th birthday.
  • and 2 doses of varicella (chickenpox), both doses given after 12 months of age, or healthcare provider verified "had disease".  

Without proof of these additional immunizations, your child will not be allowed to attend school.

Please bring your child's completed official immunization record to the school office so that our records may be updated.  If you have presented proof previously, this will satisfy the requirement.

Personal belief exemptions are no longer an option for vaccines required for school entry as of 1/1/2016 (SB 277).  For students advancing to 7th grade, all personal belief exemptions will expire. 

Medical exemptions will be accepted. A medical exemption is a written statement from a medical doctor (M.D.) or a doctor of osteopathic medicine (D.O.) which states (1) the child's name and date of birth; (2) specific nature of the physical condition or medical circumstance of the child for which a licensed physician does not recommend immunization; (3) each specific required vaccine that is being exempted; (4) and whether the medical exemption is permanent or temporary.  Temporary medical exemptions will expire 12 calendar months from the date of signing.
